PRESS RELEASE

INCIDENT: Arrest made by the Scituate Police Department for the Fatal Motor Vehicle Accident that took the life of Mrs. Katherine C. O’Toole, 48-year-old mother of six children from Foster, Rhode Island.

Where/When: At the Scituate Police station 7:40am Thursday, April 9, 2009.

SYNOPSIS: Mr. David W. Hazard DOB 04/14/1971 of 354 Burnt Hill Road, Scituate, RI, who was the operator of the 2003 Dodge Ram pickup truck turned himself in at the Scituate Police Station.

Mr. Hazard was charged with one Felony Count: Driving Under The Influence Death Resulting.  Mr. Hazard was also issued a Traffic Summons for operating left of center, crossed double yellow lines, operating on and expired registration, expiring March 31, 2009 and operating without evidence of insurance.

Mr. Hazard was arraigned by Judge Jeanne LaFazia at Third District, Kent County Court House, Warwick, RI at 11:30am this date and placed on $25,000 surety bail with substance abuse and alcohol screening next court date appearance June 18, 2009 for arraignment in Providence, RI, Superior Court.
  
 The Scituate Police Department is working in conjunction with the Rhode Island Attorney Generals Office, on this Fatal Motor Vehicle Accident/Investigation, which is still on going. More criminal charges are expected in the immediate future.
