CICILLINE TO PARENTS: Sign Swine Flu Vaccine Consent Form for Your Kids

PROVIDENCE, R.I. (AP) - Providence Mayor David Cicilline wants parents of school children to sign consent forms so the students can immediately receive the vaccine against swine flu.

Students can get the vaccine at school-based clinics starting
Monday. Cicilline says the state Health Department can't administer the vaccine without a signed consent form.

Forms were mailed to parents two weeks ago.

The mayor's statement Thursday comes three days after the death of a 12-year-old Bristol girl who tested positive for the H1N1 strain of swine flu.

Not everyone got a form! I had to download mine from the Prov School Dept website. I'm still waiting for the 'form' to come in the mail, perhaps other parents are 'waiting' too.
