On Friday, August 21, 2009, at 2:58 AM, the Lincoln Woods Barracks received numerous calls reporting a motorcycle accident. Troopers were dispatched to the scene of the accident on I-95 South in the area of Exit 16 in the City of Providence.

Upon troopers' arrival at the scene, it was determined that the crash had resulted in one (1) fatality. Members from the RI State Police Criminal Identification and Accident Reconstruction Units were called to process the scene.

Further investigation revealed that three (3) motorcycles were traveling South on I-95 in the area of Exit 16 in the City of Providence. Two (2) of three (3) motorcycles were involved in the crash. A witness reported observing these motorcycles to be traveling at a high rate of speed just prior to the crash.
The two (2) motorcycles involved in the collision had back seat passengers who were ejected onto the pavement on Route 95 approximately one tenth of a mile south of the exit to Elmwood Avenue. Both of these passengers were wearing protective motorcycle helmets; the operators were not wearing helmets.
The operator of motorcycle #1 has been identified as William Pagliarini, age 27, of 315 Smithfield, North Providence. Mr. Pagliarini was transported to RI Hospital for minor injuries and later released.

The passenger of motorcycle #1 has not yet been identified pending positive identification by the Medical Examiner's Office.

The operator of motorcycle #2 has been identified as Fernando Holguin, age 20, of 1149 Elmwood Avenue, 3rd floor, Providence, RI. Mr. Holguin was transported to RI Hospital with non-life threatening injuries.

The passenger of motorcycle #2 has been identified as Catarina G. Pacheco, DOB age 18, of 24 Whipple Street, 1st floor, Providence, RI. Ms. Pacheco was transported to RI Hospital and is currently in critical/stable condition.

The operator of motorcycle #3, who was not involved in the crash, has been identified as Robert Pagliarini, age 20, of 52 Coggeshelle Street, 2nd floor, Providence.
