An investigation into the death of a man during an aerial lift accident in Taunton has determined it could have been prevented.

OSHA say that the August 18 accident that killed 48-year-old Kevin Miranda could have been prevented if the lift was on level ground. They also say Miranda’s fall protection lanyard was not attached to the basket or boom.

Miranda was working for the Contracting and Roofing Corp. when his lift was extended to a height of 45 feet when it tipped over. He was taken to Morton Hospital where he died from his injuries.

OSHA cited Skyline Contracting and Roofing Corp. for two willful violations and one serious violation. The proposed fines total $102,900.
