Cognitive Impairment Indicator Form now available from Blackstone Police

BLACKSTONE, Mass. (WLNE) — The Blackstone Police Department said that it has launched a new initiative for residents with cognitive impairments.

The Cognitive Impairment Indicator Form now available on the department’s website can be filled out by any resident on behalf of a loved one with a cognitive impairment, such as autism or Alzheimer’s, and can include a photo.

The filled form then goes to the chief as well as patrol officers and dispatchers, so they will know that individual has special needs.

Chief Gregory Gilmore said the initiative came about when a mother of  an autistic child reported her child missing, and after he was found urged the department to take steps to keep the cognitively impaired safe.
